Thanks for reporting this. This ocx file is responsible for the main menu drawing. I think that there is a new version available where this is possibly fixed. Have you tried to reinstall Diving Log?

Normally it doesn't matter if UAC is on or off, Diving Log runs well with UAC on. The comdlg32.ocx is a shared runtime file for the file open / file save dialogs. So maybe another software on your computer replaces or deletes this file. Have you tried to copy the Vista file into the Diving Log progr...

I know, the template function can be dangerous if you overwrite your data. So maybe some more warnings would be a good idea, you're right. I also recommend when using the template function to use also the backup feature. So you have a chance to restore your logbook in case of a mistake.
